Pregnancy is an exciting journey, and having a trusted healthcare team by your side can make all the difference. Through our Share Care model, Lucas County Health Center partners with MercyOne and UnityPoint Health – Des Moines to make quality prenatal and postnatal care more accessible to families in our community.
With convenient appointments close to home, you can receive much of your pregnancy care right here in Chariton while having a coordinated plan in place for delivery.
What is Share Care?
The Share Care model allows you to receive prenatal care at LCHC Medical Clinic with Dr. Greg Cohen or Sharon Squibb, Certified Nurse Midwife. From your first positive pregnancy test through the later stages of pregnancy, our team is here to support you and help you prepare for your baby’s arrival.
Your prenatal care includes many of the same services you would receive at a larger facility, including:
- Routine prenatal appointments
- Ultrasounds
- Laboratory testing
- Pregnancy education and guidance
- Ongoing monitoring of your health and your baby’s development
You can begin receiving care as early as your first positive pregnancy test, helping you get your pregnancy off to a healthy start.
Planning for Delivery
You can continue receiving prenatal care in Chariton until approximately 36–38 weeks of pregnancy. As you approach delivery, Dr. Cohen or Sharon will work with you to identify the provider best suited for your delivery and coordinate a personalized care plan for you and your baby.
Our goal is to make the transition as smooth as possible while ensuring you feel informed and supported every step of the way.
Care Close to Home
The Share Care model helps reduce the need for frequent long-distance trips for prenatal appointments, saving you time away from work and family. You can enjoy the convenience of receiving much of your pregnancy care close to home, with a coordinated plan for delivery and continued support after your baby arrives.

Postnatal Care for You and Your Baby
Your care doesn’t end when your baby arrives! After delivery, you can return to LCHC Medical Clinic for postnatal care and support for both you and your baby.
Services include:

- Guidance on postpartum recovery and what to expect after birth
- Education about baby blues and postpartum depression
- Birth control options
